Appointments |

Call
us at (972) 937-8900 during office hours to schedule
appointments.
We
make every effort to stay on time, but some patients have
complex or unexpected problems that require additional time.
Emergencies also arise that must immediately be cared for. If
you have a medical problem you feel is urgent, please make this
clear to the receptionist when you call. |

Your
First Visit |
|
|
When
you arrive please check in at our reception desk to fill out an
information form. This gives us information about you and helps
us care for you in the best way possible.
On
visits after this first time, please sign in at the reception
desk when you arrive. This lets us know you are here and ready
to be seen.
If
you have been referred by a doctor, a coach, trainer or
therapist, please let us know at your first visit so we can send
them a report of your visit. Bring to this first visit any
X-rays or MRI records or other records that you have related to
your condition. |

Letters and
Narrative Reports |
|
|
We
will complete letters and reports necessary for insurance or
workers’ compensation claims at your request and with your
written authorization. There is a fee for this service based on
the time required to complete the documents. |
|
|
|
|
Appointment
Cancellations |
|
|
Notify
the office in advance if you must cancel your appointment so
another patient can use your time. |
|
|
|
|
Prescriptions
Refills |
|
|
Your
pharmacy needs to contact us for refills. If you need a
prescription refilled, please contact the pharmacy before you
are completely out of your medication. You must first, call your
pharmacy to request a refill, and the pharmacy will contact our
office for authorization. We fulfill authorizations for refills
during these hours:

Monday – Thursday, 8:30 a.m. – 4:30 p.m.
*
Friday, 8:30 a.m. – 2 p.m. Requests after 2 p.m. Fridays will
be processed on the following Monday after we have had an
opportunity review your records. |

Protected
Health Information |
|
|
This
practice uses and discloses health information about you for
treatment, to obtain payment for treatment, for administrative
purposes, and to evaluate the quality of care that you receive.
Please read our "Notice of Privacy Practices" for
further information on how we use and release this information. |

Your Medical
Record/X-rays |
|
|
Your
medical record is strictly confidential. The medical record is
the property of this office and cannot be removed. You may
receive a copy, and there is a fee for reproducing the record.
X-rays are considered part of your medical record, and there is
a fee for reproducing these as well.
To
keep your records accurate you let us know if any personal
information changes, including name, address and phone number
changes. |

Emergencies |
|
|
If
you have a life-threatening medical emergency and need immediate
medical attention, CALL 911. If this is an urgent problem, call
our office at (972) 837-8900. |

|
|
Fees and
Payment |
|
|
We
share the same concerns you have about the high cost of medical
care. Please be assured that this clinic’s fees are average
for our area. If you have any questions about the fees, we are
happy to discuss them with you.
Payment
is required for office services on the day you receive your
care. For those whose care is covered by insurance, this refers
to your co-payment. We are happy to process your insurance
claims. We accept cash and check payments as well as MasterCard,
Visa and Discover. |

Insurance |
|
|
We
participate in most health plans, and we are happy to file your
insurance claim for you. It is your responsibility to be aware
of the coverage limitations of your own plan. Co-payments and
insurance deductibles are to be paid at the time of your visit.
Workers’ Compensation payments also are accepted upon
approval. |

|
|
|
Medicare |
|
|
We
are happy to help our Medicare patients with their claims. We
accept assignment on Medicare. Patients are responsible for
co-payments and deductibles. |
|
|
|
Surgical
Fees |
|
We
care for patients at both outpatient surgical centers and at the
hospitals where we have privileges. Surgery patients will
receive a billing from the hospital or surgical center for that
service, as well as a bill from our office for the care we
provide. You are responsible for any fees not covered by your
insurance plan, and payment is due within 60 days. |

Past Due
Accounts |
|
If
your account becomes past due and you are having difficulties
making payment, call us to make arrangements so that no
collection action becomes necessary. We will be happy to work
with you in any way we can. |
